REPORTING A SUSPENSION
The ODMHA code of discipline, Item 9.0 c) of the ODMHA handbook, defines the Coach’s role pertaining to suspensions. The following explains this procedure in District 4.
COMPETITIVE TEAMS MUST
Report any Code of Discipline offence that results in the player/official being removed from the game and confirm the discipline (if any) to be assessed (number of games to be served).
WHAT TO REPORT
CONTENTS OF REPORT Include the following information:
Your name, position on the team and a call back number.
- The penalty received as shown on the game sheet, including:
- date of the game;
- player/official's name & number or position;
- penalty/penalties assessed;
- period and time of each penalty;
- your interpretation of the discipline to be applied (as per ODMHA, Code of
Discipline); and
- other information (if necessary) - 1st fight, 2nd, 3rd, etc., league, playoff,
exhibition or tournament game, need confirmation by ______ next game.
- Dates the discipline (suspensions) will be served.
WHEN TO REPORT Within 24 hours of the game played or within 24 hours of return from out of town
tournament.
HOW TO REPORT
Report the incident by online, email, fax and/or phone:
- OnLine www.district4.ca (preferred method) - the system allows you to report up to 5 codes, if you there are more then enter another report. Fax game sheet to 1-888-250-8482.
- Email suspensions@district4.ca
-Include the contents listed in #2 and a scanned copy of the game sheet or fax the game sheet to 1-888-250-8482, as soon after sending the email as possible.

- Include a typed (or neatly hand written) cover page with the contents listed in #2 and the game sheet(s). If the submission can not be read, it can not be acknowledged.
- Phone 1-888-490-0708
- All phone calls MUST be followed up with either the email or fax.
- Use this only when clarification or a turn around in less than 24 hours is required, e.g. game next day or in a tournament.
- Calls after 9:30 pm will typically be returned the next day; please make sure to leave a return phone number in the voicemail with the details.
SERVING A SUSPENSION: COMPETITIVE TEAMS
GAME SHEET
- Ensure that the name of the individual(s) serving a suspension is noted on the game sheet.
- Ensure that the referee initials the individual(s) name.
- Ensure game in the suspension is being served, e.g. 1 of 3, is included.
- Only regular season; tournament and playoff games are considered for serving a suspension.
- Keep your copies of ALL game sheets showing suspensions being served till December of the following season.
INDIVIDUAL
GAME SUSPENSION
- not allowed to participate with team in any game (regular season; exhibition; tournament or playoff)
- only regular season, playoff and tournament games are considered when counting games served
- not allowed to be with the team before or after the game the suspension is being served, only allowed in the
public areas of the arena
TIME SUSPENSION
- Not allowed to participate with the team in ANY event or game till the suspension has been served.
PENALTIES FOR NOT SERVING A SUSPENSION
- Severe suspensions are in place for coaches who fail to sit players/officials out the proper suspension.
- Some competitive leagues have a provision for forfeiture of game for playing players not eligible.
REPORTING SERVED SUSPENSIONS: COMPETITIVE TEAMS
INFORMATION TO REPORT
- Regular Season
- Send an email to suspensions@district4.ca indicating the player (name and number).
- Retain the game sheet, you may be asked to submit (fax) a copy.
-Reviews will be randomly done through out the season, so make sure to retain your copies of the game sheets.
- If you are unable to demonstrate that a suspension was served, it will be considered not to be served.
Playoffs
- Send an email to suspensions@district4.ca indicating the player (name and number).
- Include a scanned copy with the email or fax the game sheet to 1-888-250-8482 with a cover sheet indicating
“Suspension # of # served by PLAYER NAME” and your name and call back number.
- Game sheets for all served suspensions must be submitted.
-These are required to determine if there will be any carry over suspensions to the next season.
REPORTING TO LEAGUE / ASSOCIATION
- A/AA leagues & B league and clubs require coaches to report suspensions to the league / club.
-This reporting is secondary to the District.
-Include a “cc” to your league coordinator on your email to District 4 ( suspensions@district4.ca ).
